Congratulations to all of our clients’ who have fallen pregnant. Mary and I couldn’t be happier for you. I thought that I would take a bit of time and share some information along with some helpful tips that will support your skin during the next 9 months as your body’s anatomy undergoes significant changes that may have an impact on your skin. TIP 1: Avoid direct sun exposure on your face! Why? Pregnancy has a significant impact on your hormones and causes a major increase in progesterone, with the placenta turning out 10-20X higher than what is normally experienced during a regular menstruation cycle. Progesterone increases the activity of the Melanin-Stimulating-Hormone (MSH) which triggers the melanocyte (pigment cell) to produce significant amounts of melanin which can result in pigmentation disorders. Estrogen when out of balance can also give rise to pigmentation so if you are experiencing signs of pigmentation during your pregnancy, and you know you’re staying out of the sun, don’t panic. Once the cause has been removed IE your baby, the skin should return to ‘normal’ if you have not overexposed your pigment cells to significant amounts of ultraviolet radiation during this time. What can you do? Consult with your Corneotherapist about updating your prescription and adding in tyrosinase inhibitors that can slow down the transfer of melanin within the skin as well as wearing sunscreen, polarised sunglasses and solar barrier hats and clothing to protect you. We recommended solbari.com.au TIP 2: Why am I experiencing acne during my pregnancy? Answer: Some woman experience beautiful skin during their pregnancy, while others have the complete opposite. You must understand every cell in the skin has hormone receptors and these particular receptors or ‘docking stations’ as I call them may be having a sensitivity and/or intolerance to the overabundance of progesterone that is being bestowed upon them; altering the quality and quantity of your sebaceous secretions (oil) and inflicting poor corneocyte compaction which can lead to congested and acneic skin. What can you do? This is rather challenging as it is an intrinsic factor, however, one can most definitely support their skin with physiological skincare preparations, 5a Reductase inhibitors (consult with your Corneotherapist) and enzymatic exfoliation instead of ablating the skin with products and/or modalities that compromise skin barrier defence and make acne worse! Take our word for it; don’t have ANY microdermabrasions or chemical peels. Consulting with a Naturopathic Practitioner is also advised during this time to support your hormones. TIP 3: What can I do about stretch marks? Answer: Daily massage with skin nurturing substances such as our dermaviduals Body Lotion and Wheat Germ Oil can really help to improve the elasticity of the skin as the dermis (collagen layer) begins to stretch as your anatomy changes. SCENAR Training We have just completed 4 days of SCENAR level 1 and 2 training. It was very intense but very helpful and we’re looking forward to treating more clients with this technology. We don’t use a Western medicine approach but instead treat the whole body, physical, emotional, mental and spiritual. The technology is from Russia and is like a TEN’S machine but it has a biofeedback mechanism which means your body cannot become accustomed to it like it will with a TEN’s. The only contraindications are if you have a pacemaker, if you are an epileptic or have an infectious skin disease. We’ve already treated many clients successfully and want to help more people who have either chronic or acute pain. I had tendonitits in both of my arms from too much gardening and I have treated that and have no symptoms now. Other clients are now no longer requiring pain medication. There is a delicate balance between keratinocyte migration from the basal cell layer (birthing layer) to the stratum corneum (top layer of skin) which in retrospect, is deemed the roof of your house or barrier to the largest organ of your body. Corneocytes (roof tiles) are shed from the surface of the skin which arise newly stratified squamous keratinized epithelium; all while keeping consistent thickness of the stratum corneum relatively intact. Desquamation also treads a fine balance between adequate breakdown of the barrier to allow a continual renewal of keratinocytes and leaving the barrier sufficiently intact to prevent allergens and irritants from penetrating through to the deeper layers of the skin. Microdermabrasion not only removes roof tiles, but important lines of skin barrier defence such as the acid mantle/microbiome (oil film) and multi-lamellar lipid structure (oil secretions), all in which can take several days to recover post treatment. The stratum corneum is only two-hundredths of a millimeter thick so by performing a microdermabrasion treatment, you remove almost all of it. The stratum corneum is composed of around 15-20 sheets of unseparated corneocytes; all playing a role in cell-to-cell communication while providing effective physical and water barrier functions in the skin by slowing trans-epidermal water loss (TEWL). The corneocytes are the first cells to activate the innate immune system, so any disturbance to this barrier function will promote cellular inflammation; and cellular inflammation contributes to the premature ageing process and disease. Keratinocytes (epidermal cells that make your roof) undergo a process of terminal differentiation where they give rise to the corneocytes that make up the stratum corneum. Corneocytes contain water molecules (NMF) which help to maintain skin hydration and are protected by a layer of lipid lamellar which encases the corneocytes; preventing high TEWL and impeding barrier permeability. Corneocytes are tethered together by corneodesmosomes (connecting bonds) that anchor into the sides of each corneocyte for proper roof tile alignment by means of preventing your roof from falling off. With this explanation in mind, I want you to picture your startum corneum (roof) like a brick wall. The corneocytes are the bricks and the lipid lamellar encases the bricks in exactly the same way mortar does. Most people believe the surface of their skin is covered by a veil of dead, useless cells that couldn’t possibly have any purpose at all. They believe that by scrubbing them away with rough, granular substances, they can make the skin bright and fresh again, however, in reality, these so called ‘dead’ skin cells (corneocytes) are in fact the mature functional cells of the skin. They are there for a special purpose known for external barrier protection; enabling cell-to-cell communication and defending against undesirable microorganisms or the sun from penetrating and causing cellular inflammation. I understand post a microdermabrasion treatment that your skin may feel as though it’s healthy and glowing, however, the truth is that you have exposed immature keratinocytes to pollution, rays and oxygen long before mother nature intended to cope with these hazards. These immature keratinocytes are shocked by this sudden exposure and cannot truly perform their function of protection because they themselves require protection from the corneocytes and lamellar lipid structure which have all now been scrubbed away. GALA TIME! Kai and I will be in Sydney this Sunday, Monday and Tuesday for the dermaviduals Symposium and Gala Ball. Kai is a finalist in the junior corneotherapist of the year awards so we are hoping he will return home with another certificate and a trophy. We know many of you have supported in him by writing some beautiful testimonials for him. Let’s keep our fingers crossed that Tassie can win this award. I had to take some time off as I developed facial shingles. Shingles (Herpes Zoster). This is a reactivation of varicella-zoster virus. Anyone who has previously had chickenpox may develop shingles. The virus stays dormant in dorsal root ganglia nerve cells in the spine. Once it is reactivated it migrates down sensory nerves to the skin to cause shingles. It can occur in children but is more common in adults. Luckily I only had a very mild dose with 5 spots and no eye nerve involvement and no neuralgia pain but the most intense itching. Shingles (Herpes Zoster). This is a reactivation of varicella-zoster virus. Anyone who has previously had chickenpox may develop shingles. The virus stays dormant in dorsal root ganglia nerve cells in the spine. Once it is reactivated it migrates down sensory nerves to the skin to cause shingles. It can occur in children but is more common in adults. It appears as a blistering localized painful itchy rash on one side of the face or body. It is more common on the face than the body. It can be spread by close physical contact to those who have NOT been infected with or vaccinated against the virus. It remains contagious until the blisters have healed over. Once the blisters are broken and scabbed over or are well covered, they cannot spread the virus. It often affects people with a weakened immune system. Complications of shingles if left untreated is post herpetic neuralgia (intermittent pain and intense itching along the course of a nerve) eye infection, loss of vision, glaucoma, facial palsy (paralysis) ear ache, hearing loss, dizziness, loss of taste and an increased risk of bacterial infection. Diagnosis is made by pain history, a physical examination by your doctor and a swab can be taken. You may also advised to see an ophthalmologist if the shingles are around your eye to rule out any nerve involvement. (Mine was not involved). Although the virus cannot be cured anti viral medication can reduce the severity of the symptoms and shorten the duration of the illness. These include acyclovir (Zovirax), valacyclovir (Valtrex), and famciclovir (Famvir). Cool compresses, calamine lotion, capsaicin cream for the neuralgia and avoiding scratching or popping the blisters are some of the recommendations for treating shingles. Personally I have found the intense itching to be the worst part of my journey. I am taking anti-viral medication and a supplement to boost my immune system. I’m also resting, drinking lots of water, eating healthy foods and not seeing any clients until next week. The Melanocyte originated from embryonic development (specifically the Neural Crest) and is located throughout the body where pigment can be seen such as the hair, skin and eyes. As a skin treatment therapist specialising in corneotherapy, I am briefly going to discuss epidermal pigmentation in two parts. Located on the Basal Layer of the epidermis, the melanocyte is adjacent to 10 keratinocytes (cuboidal cells) and can make contact with up to 30-36 different keratinocytes by releasing melanosomes (pigment granules) via their long branching extensions (dendrites) to be received by the keratinocytes for cellular protection against UV radiation. When melanosomes are being manufactured inside the Golgi Apparatus of the melanocyte, the pigment granules are completely colourless until immediate contact with the keratinocytes PAR-2 Receptor takes place. Melanosomes are programmed to settle on top of the nucleus of the keratinocyte to protect the individual’s genetic material from the sun; think of these pigment molecules as a fundamental sunhat. The keratinocyte will continue its programmed journey of differentiation towards the Stratum Corneum until it will finally desquamate as a corneocyte; taking the pigment granules along with it. Melanin begins formation when there is direct UV radiation exposure; whether direct skin-to-skin contact or through the retina of the eye, the melanocyte is programmed to make the pigment carrying melanosome to protect and defend the body. If the melanocyte has received too much sun exposure, this will result in DNA damage to not only the melanocyte, but also to the keratinocyte cell membrane and result in the overproduction or underproduction (depending on the level of cellular damage) of pigment granules to the surrounding keratinocytes. Remember, the melanocyte has access to 30-36 different keratinocytes and the ratio of melanosomes towards these cells can be altered and result in pigmentation disorders. Damage to the melanocyte cell membrane causes the dendrites to become less flexible and viable; compromising the melanosome transfer from melanocyte to keratinocyte and this can result in pigmentation. It’s important to understand the melanocyte is a cell that does NOT desquamate which makes it very difficult to treat. Please note, if the keratinocyte mother cell (stem cell) becomes impregnated with melanosomes, that individual is going to have to live with the damage. Melanosomes can also filter down into the Dermal/Epidermal Junction (connective tissue) and overtime, can become visible on the skin as this junction contains many intricate channels for pigment granules to accumulate. Cellular damage (ageing process/UV radiation) causes the Dermal/Epidermal Junction to collapse and the pigment granules are released into the dermal matrix; resulting in dermal pigmentation. The melanocyte has an excellent memory and if its programmed manufacturing of melanosomes becomes compromised in anyway, the individual has to live with the damage. There are modalities available to treat this condition, however, the minute your skin or eyes has direct UV radiation exposure, BANG! All of your hard work to inhibit the formation of pigment becomes undone and as skin treatment therapists say in this industry, prevention is always better than cure.
